Filling out this simple form makes it possible to present you with your ARC Completion Card upon successfully finishing the Course (if filled out at least a week before class).  (Failure to submit data card results in snail mail delivery of your Completion Card
usually 4 weeks after graduation)

This is what you want us to call you during your training. Use a nickname, first name, last name -- whatever you want your fellow riders to call you
Asked solely to keep groups gender-balanced if possible. (Most names are gender specific, but some are not -- Casey, Addison, Ryan, or Leslie -- works best to ask and confirm.)
