DO NOT submit this form unless your USR/UMSR
pre-registered you for a
ARC (Advanced Rider Course)

Submitting this form when not pre-registered through your USR/UMSR
makes your registration void — without notification — meaning you
are not part of the program nor will you receive any messages to
let you know! Make sure you do it “By the Numbers”
found on the Mil HomePage. Talk to your
USR/UMSR before filling out this form.

RidingSafety is under contract to the AF.  None of the
information below is released or shared outside program.

A commercial email is required. Emails sent to the af.mil addresses are sometimes returned undeliverables for reasons unknown. If you do not provide a commercial contact RidingSafety may not be able to communicate with you unless commercial email is provided.
(Sponsor's name found UNDER your name on MSF BRC/ERC Completion card. Need Sponsor, not instructor's name. Please don't give name of person who trained you. Incorrect Sponsor name will require you to fill form out again with requested information.)
